This Policy applies to information we collect through:

- SMS/text messaging programs you opt into;
- Client intake, assessment, scheduling, and service delivery activities;
- Job applications and employment onboarding.

Identifiers & contact details. Name, address, email, phone numbers, and emergency contacts.
Service & clinical information (PHI). Assessments, service plans, visit notes, schedules, communications with your care team, and other information needed for care coordination and billing.
Demographics & eligibility. Age, disability-related information you choose to share, and payer details (e.g., MaineCare).
Employment applicant data. Resume/CV data, work history, credentials, and background-check results as allowed by law.
Website/technical data. Log files, IP address, device/browser info, and limited cookies necessary for site performance and security.
SMS program data. Your opt-in status, message preferences, and delivery/response metadata.

- Care delivery & coordination (Treatment). To assess needs, plan and provide services, and communicate with your care team.
- Billing & administration (Payment/Operations). To verify eligibility, bill payers, manage quality, train staff, and improve services.
- Scheduling & reminders. To send appointment confirmations/updates (SMS/email/phone).
- Hiring & HR. To recruit and onboard employees and meet workforce/credentialing rules.
- Safety & compliance. To meet reporting obligations (e.g., abuse/neglect), respond to audits/surveys, and maintain secure records.
